What this website collects

This website does not collect, transmit, or store personal health information. The patient registration form prepares an email in your own email app, addressed to the clinic — you review and send it yourself, and nothing you type is stored on this website or shared with any third party. We do not use advertising trackers.

When online registration launches, it will be handled by OceanMD, a PHIPA-compliant patient engagement platform hosted in Canada. This page will be updated before that change takes effect.

How the clinic protects your health information

As a health information custodian in Ontario, St. George Medical Center collects, uses, and discloses personal health information in accordance with the Personal Health Information Protection Act, 2004 (PHIPA). Your records are used to provide you with care, are shared only with those involved in your care or as permitted by law, and are protected with administrative, technical, and physical safeguards.

Your rights

You may request access to or correction of your health record, and you may withdraw or restrict consent to certain uses of your information, subject to legal exceptions. You may also direct privacy complaints to the Information and Privacy Commissioner of Ontario.
